Living Well With Chronic Conditions
Living Well with Chronic Conditions is designed to improve the self-management skills of people living with ongoing health issues. Conditions like arthritis, mental health conditions, chronic pain, cancer, or diabetes can cause those affected to lose physical conditioning and suffer health problems over many years. This program teaches new strategies that will give participants the confidence, motivation, and skills needed to manage living with a chronic health condition.

onnect with others and review topics like action planning, nutrition, making treatment decisions, and communicating with health care providers and family. Toolkits and group call information will be sent to participants after registration.
NOTE: There are six sessions in this workshop. The session that would normally take place on December 28 will be delayed by one week. The workshop will resume the following week, on January 4, and the last session will take place in the 7th week on January 11.
