Living Well With Chronic Conditions
Living Well with Chronic Conditions is designed to improve the self-management skills of people living with ongoing health issues. Conditions like arthritis, mental health conditions, chronic pain, cancer, or diabetes can cause those affected to lose physical conditioning and suffer health problems over many years. This program teaches new strategies that will give participants the confidence, motivation, and skills needed to manage living with a chronic health condition.

pression, anxiety, diabetes, heart disease, and high blood pressure. Take control of your health with a six-week interactive workshop that can help you:
• Find useful ways to deal with pain and fatigue
• Learn about making small changes toward healthier eating
• Find safe and effective ways to start of increase exercise
• Learn self-management skills to live life to the fullest
• Learn how to effectively communicate with your doctor and family
• Set your own goals and make a step-by-step plan to improve your health and your life
Participants will meet ONLINE / virtually via Zoom weekly for two and a half hour sessions for six consecutive weeks. There is no charge to attend. We will hold a "session 0" where participants can meet one another, practice using Zoom, and receive support for technical challenges. This session 0 will be held on Wednesday, January 19th at 9:30AM. Registered participants will receive the Zoom link for the session 0 and each class session via email.
