This program teaches new strategies that will give participants the confidence, motivation, and skills needed to manage living with chronic pain. Classes are highly participative, where mutual support and success build the participants' confidence in their ability to manage their health and maintain active and fulfilling lives.

Living Well with Chronic Conditions is designed to improve the self-management skills of people living with ongoing health issues. Conditions like arthritis, mental health conditions, chronic pain, cancer, or diabetes can cause those affected to lose physical conditioning and suffer health problems over many years. This program teaches new strategies that will give participants the confidence, motivation, and skills needed to manage living with a chronic health condition.

The Diabetes Prevention Program is for people who want to avoid type 2 diabetes. It uses a curriculum developed by the Centers for Disease Control and Prevention and cuts your risk of developing type 2 diabetes in half. The program helps you make lasting changes, such as eating healthier, adding physical activity into your daily routine, and reducing stress.

Most Medicare and Medicaid plans are covering the costs of these programs, as well as some commercial insurance plans. For those without insurance coverage, contact class leader for more information. informational sessions will be on Tuesday October 15th & again on Tuesday October 29th, from 1-2 pm. Need to only attend one.

People with type 2 diabetes attend the class in groups of 12-16. Classes are facilitated from a highly detailed manual by two trained Leaders, one or both of whom are peer leaders with diabetes themselves. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Physicians, diabetes educators, dietitians, and other health professionals both at Stanford and in the community, have reviewed all materials in the class.

The Diabetes Prevention Program is for people who want to avoid type 2 diabetes. It uses a curriculum developed by the Centers for Disease Control and Prevention and cuts your risk of developing type 2 diabetes in half. The program helps you make lasting changes, such as eating healthier, adding physical activity into your daily routine, and reducing stress.

Individuals identified as at risk for pre-type 2 diabetes are invited to participate in Diabetes Prevention Program (DPP) group classes, which consist of 12 to 16 participants. Each session is led by two qualified instructors, one of whom is typically a peer leader with personal experience in managing diabetes. Participants are encouraged to develop weekly action plans, share their experiences, and collaboratively address challenges related to their self-management efforts. The course materials have been thoroughly reviewed by physicians, diabetes educators, dietitians, and other healthcare professionals associated with Stanford and the local community. IMPORTANT: PLEASE READ BEFORE REGISTERING FOR THIS CLASS This workshop will be conducted in a virtual format. A fundamental understanding of computer skills is crucial for all participants. Before starting the class, participants must install the Zoom platform on their devices, ensuring they have access to equipment with camera and audio capabilities. For optimal participation, we recommend using a tablet, laptop, or desktop computer rather than a mobile phone. Before registering, please complete the pre-class survey, consent to the insurance, liability, and privacy policy forms, and submit your most recent A1C readings to confirm eligibility for the DPP Medicare beneficiaries. The submission of these documents is essential for gaining access to the virtual class and its accompanying materials. Each participant will receive either a hard or digital copy of the course textbook, which will be mailed via the United States Postal Service once AFROCARE SERVICES has received all necessary registration documents. We appreciate and recommend a donation of $50 per participant for class materials, although the actual value of these classes is $170.00 per individual. Powerful Tools for Caregivers® helps family and friends caring for older adults with long-term health conditions develop the skills and confidence to better care for themselves while caring for others. The weekly topics range from reducing your stress to communicating in challenging situation and mastering caregiving decisions.

Registration is open to the general public (caregiving for individuals age 60+) by January 15th. A Session 0 will be held on January 15th, to allow participants to test their Zoom link. Zoom link and class materials will be mailed to participants prior to the workshop start date.
