As a healthcare provider, you are part of the backbone of Minnesota’s care delivery system. You regularly make referrals to services that help your patients thrive. We encourage you to include Juniper classes in your mix of referrals.
Juniper offers supportive, educational programs for patients with chronic pain and disease and those who would benefit from increased physical activity. Classes are evidence-based (see clinical summaries below) and support and extend the care you provide.
Our programs can help you achieve care management goals and produce better quality outcomes for your patients.
E-learning module on referring patients to Juniper programs
We are excited to share an e-learning module from the Minnesota Department of Health on how to refer patients to Juniper evidence-based disease prevention and self-management courses. Developed in partnership with the Health Promotion Chronic Disease Division and the Office of Statewide Health Improvement Initiatives. The course is free and offers .5 CEUs. Access the course, create an account, and then search "Juniper." The course is titled "The Juniper Solution: Referring Patients to Evidence-Based Disease Prevention and Self-Management Programs".
