Tai Ji Quan: Moving for Better Balance® (TJQMBB) is an evidence-based fall prevention program designed for older adults developed by Fuzhong Li, Ph.D., Senior Scientist at the Oregon Research Institute. TJQMBB represents a substantive enhancement of traditional Tai Ji Quan as it transforms martial arts movements into a therapeutic regimen aimed at improving postural stability, awareness and mindful control of body positioning, functional walking, movement symmetry and coordination, range of motion around the ankle and hip joints and lower - extremity muscle strength.

The Diabetes Prevention Program is for people who want to avoid type 2 diabetes. It uses a curriculum developed by the Centers for Disease Control and Prevention and cuts your risk of developing type 2 diabetes in half. The program helps you make lasting changes, such as eating healthier, adding physical activity into your daily routine, and reducing stress.

Please read the following information carefully before registering for this class. The Diabetes Prevention Program (DPP) helps people with prediabetes prevent type 2 diabetes through healthier eating, stress reduction, and increased physical activity. Sessions: The Diabetes Prevention Program (DPP) offers classes for participants at risk for pre-type 2 diabetes led by trained lifestlye coach. Participants create weekly action plans, share experiences, and tackle self-management issues. Program Format: This workshop will be conducted in a virtual format, and it is essential for all participants to have a fundamental understanding of computer skills. Prior to attending the class, participants are required to install the Zoom application on their devices and ensure their equipment has both camera and audio capabilities. For optimal engagement, it is recommended to use a tablet, laptop, or desktop computer rather than a mobile phone. Eligibility: Individuals diagnosed with prediabetes who are 18 years or older, have a body mass index (BMI) of 25 or higher (23 or higher if Asian American), have not been previously diagnosed with type 1 or type 2 diabetes, are not pregnant at the time of enrollment, do not have end-stage renal disease (ESRD), and have never participated in Medicare DPP before. In addition, the individual received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test. and had a blood test result in the prediabetes range within 12 months of the first session (includes any of Hemoglobin A1C: 5.7–6.4%; Fasting plasma glucose: 100–125 mg/dL or Two-hour plasma glucose (after a 75 g glucose load): 140–199 mg/dL. And not previously diagnosed with gestational diabetes (allowed for CDC recognition and may be self-reported; not allowed for Medicare DPP participants). Registration requirement: Before registering, it is imperative that you complete the following items: 1) Registration form 2) Pre-class survey, the 3) Prediabetes Risk Test, 4) Insurance Authorization and Release of Information, 5) Release and Waiver of Liability Agreement, 6) Notice of Privacy Practices and Receipt of Notice forms. And 7) Submit the most recent A1C readings to be eligibility to start attending the Diabetes Prevention Program (DPP). These documents are necessary for gaining access to the virtual class and its accompanying materials. Program Materials: A digital copy of the course textbook will be provided to each participant after AFROCARE SERVICES receives all required signed registration documents. Payments: The Diabetes Prevention Program (DPP) is an insurance benefit program. Importantly, anyone can join the program regardless of their insurance status or financial situation.
